QUALIFIED PROFESSIONAL

 SUMMARY:

LEDC receives support in coordinating and monitoring the range of services and support required to address the goals and desired outcomes of each participant. The QP supervises, integrates, and coordinates the plans of participants, as well as monitors their progress towards achieving outcomes. Furthermore, the QP is responsible for supervising paraprofessional staff, as well as overseeing the general operations of LEDC and the Group Homes.

 QUALIFICATIONS:

  • Have a Bachelor’s Degree in a human service field from a college or university and two years of full-time, post-graduate experience with individuals who have a IDD/MI diagnosis including supervision of direct care staff of at least one year.
  • Have a Master’s Degree in a human service field and one year of full-time post-graduate experience with individuals who have a IDD/MI diagnosis including supervision of direct care staff of at least one year.
  • Have a Bachelor’s Degree in a field other than human services and four years of post-graduate experience with individuals who have a IDD/MI diagnosis including supervision of direct care staff of at least one year.

 ESSENTIAL DUTIES:

Here are the responsibilities of this position:

1. Complete paperwork and assessments for new and ongoing participants.

2. Attend meetings to help create and update individual support plans.

3. Make sure individual support plans and day program curriculum are being followed.

4. Keep track of authorizations and make sure they are renewed before they expire.

5. Respond quickly to participants experiencing a crisis.

6. Meet monthly with each participant and update records accordingly.

7. Help participants transition to other services as needed.

8. Supervise staff and make sure they are following policies and procedures.

9. Make sure disaster and fire drills are being done and are up-to-date.

10. Hire and keep staff.

11. Train staff, including Innovations Competencies and other required training.

12. Investigate and report any abuse, neglect, or exploitation.

13. Review incident reports and make sure proper treatment was given.

14. Help with billing and payroll procedures.

15. Review personnel and participant records as needed.

16. Suggest updates to policies and procedures.

17. Fill in when needed.

18. Take precautions to prevent the spread of illness.

19. Keep sensitive information confidential.

20. Assist the Clinical Director and Executive Director as needed.
